By opting into our SMS Program, you agree to receive text messages regarding:

2. Message Frequency
Message frequency will vary depending on your interactions with our service. You may receive between 2-5 messages per month, or more based on your specific care plan and needs.
3. Cost and Data Charges
Standard message and data rates may apply to all SMS messages sent or received as part of this program. Contact your mobile carrier for details on rates.
4. Opt-Out Instructions
You may opt out of receiving SMS messages at any time by replying “STOP” to any text message you receive from us. You will receive one final confirmation message, and no further messages will be sent unless you opt back in.
