HHA / PCA Program

Personal care and home health aide services for adults who need help with daily activities at home.

Our largest program. Trained Home Health Aides and Personal Care Aides help clients with bathing, dressing, mobility, meals, medication reminders, and daily routines — all in the comfort of home. Every plan is led by a Registered Nurse and matched by language and background.

NHTD Waiver Program

Nursing Home Transition and Diversion waiver — community-based care for adults who'd otherwise need a nursing facility.

The NHTD waiver lets adults with chronic conditions or disabilities live independently at home or in the community instead of a nursing home. Our team coordinates service plans, helps with the application, and provides ongoing case management.

TBI Waiver Program

Traumatic Brain Injury waiver — specialized rehab and support for adults recovering from TBI.

The New York State Department of Health (DOH) Traumatic Brain Injury (TBI) waiver program provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice. Medicaid funds the program. Our care plans focus on rebuilding independence, cognitive function, and community participation.

Who it's for

  • Medicaid Recipients
  • Choose to live in the community rather than in a nursing facility
  • Be between 18 and 64 years old at the time you apply for waiver
  • Have a primary diagnosis of TBI or similar non-degenerative condition that results in deficits similar to a TBI such as stroke, or anoxia (oxygen supply loss)
  • Be assessed to need nursing home level of care
  • Have an approved plan of available services that reflects the services you need in order for you to live safely in the community

OPWDD / Pediatric Care

Care for children and adults with developmental disabilities and pediatric clients with medical complexity.

We serve children and adults eligible through the New York Office for People With Developmental Disabilities (OPWDD), plus pediatric clients with medical complexity.     • Individuals may receive assistance to live independently in their own homes or apartments, remain with family members, or reside in community-based group homes that provide varying levels of support.
  • Community Habilitation
    • Personalized, one-on-one support helps individuals develop essential life skills, increase independence, and actively participate in their communities.
  • Day Habilitation and Day Services
    • Structured programs provide opportunities for personal growth, social interaction, skill development, and community engagement in a supportive group environment.
  • Employment Supports
    • Services include job readiness training, career development, job coaching, and ongoing workplace support to help individuals find and maintain meaningful employment.
  • Respite Services
    • Temporary care and support are available to provide family members and primary caregivers with planned or emergency relief from caregiving responsibilities.
  • Self-Direction Services
    • This option empowers individuals and families to customize their services, select support staff, and manage budgets to best meet their unique goals and preferences.
  • Family Support Services (FSS)
    • Programs may include caregiver training, sibling support, recreational opportunities, family counseling, and reimbursement assistance to strengthen family caregiving efforts.
  • Clinical and Therapeutic Services
    • Individuals may access specialized therapies such as speech, occupational, physical, and behavioral therapy, as well as assistive technology and environmental modifications that promote independence and safety.
  • Care Coordination
    • Dedicated Care Managers help individuals and families navigate healthcare and support systems, access Medicaid waiver services, develop life plans, and connect with community resources.

The Office for People With Developmental Disabilities (OPWDD) offers a comprehensive range of services designed to help individuals achieve greater independence, community inclusion, and an enhanced quality of life.
