CDPAP in New York Has Changed: What S&A Unified Home Care Provides Today

S&A Unified Home Care no longer provides CDPAP fiscal intermediary services under New York State’s current CDPAP structure.
CDPAP itself has not ended. The Consumer Directed Personal Assistance Program continues in New York, but the way fiscal intermediary services are administered has changed. Public Partnerships LLC (PPL) is now the Statewide Fiscal Intermediary for New York State CDPAP, and CDPAP recipients are required to use PPL as their fiscal intermediary.
S&A Unified continues to provide a range of home care and community-based programs for individuals and families across New York City, Long Island, and Westchester.
What Changed With CDPAP in New York?
CDPAP, or the Consumer Directed Personal Assistance Program, is a New York Medicaid program designed to give eligible individuals greater control over who provides their personal assistance and how that assistance is managed.
Historically, multiple fiscal intermediaries operated within the CDPAP system. New York State changed that structure and selected Public Partnerships LLC (PPL) as the Statewide Fiscal Intermediary.
Effective April 1, 2025, PPL became the entity authorized to provide statewide fiscal intermediary services for CDPAP. New York State Department of Health currently directs CDPAP recipients to work with PPL as their fiscal intermediary.
That means home care agencies that previously participated in CDPAP as fiscal intermediaries no longer operate that function independently under the previous model.
For S&A Unified Home Care, this means CDPAP fiscal intermediary services are no longer among the programs we provide.
Did CDPAP End in New York?
No.
This is an important distinction, especially because people searching online may encounter older information about CDPAP agencies and fiscal intermediaries.
CDPAP still exists in New York. What changed is the fiscal intermediary structure.
If you are currently enrolled in CDPAP or specifically want to participate in CDPAP, information about enrollment and the current statewide fiscal intermediary should be obtained through New York State Department of Health and PPL.
If, however, you are looking for professional home care, personal care assistance or other community-based services, S&A Unified continues to provide several programs that may be appropriate depending on your needs, eligibility and insurance coverage.
What Home Care Services Does S&A Unified Provide?
S&A Unified Home Care provides several home care and community-based programs in New York. Our current programs include HHA/PCA home care, NHTD Waiver services, TBI Waiver services, and OPWDD/Pediatric Care.
HHA and PCA Home Care
Our Home Health Aide (HHA) and Personal Care Aide (PCA) program provides assistance for people who need help remaining safely and comfortably at home.
Depending on an individual’s care plan, an HHA or PCA may assist with activities such as bathing, dressing, mobility, meal preparation, daily routines and medication reminders.
At S&A Unified, HHA/PCA care is supported by an RN-led plan of care, caregiver coordination, supervisor availability and backup coverage when needed.
This model is different from CDPAP. In CDPAP, the consumer has a consumer-directed role in selecting and managing their personal assistant. With traditional agency-based HHA/PCA services, the home care agency helps coordinate the caregiver and ongoing care.
Neither model is automatically better for every family. The appropriate type of care depends on the person’s eligibility, health needs, preferences and coverage.
NHTD Waiver Program
S&A Unified also works with the Nursing Home Transition and Diversion (NHTD) Waiver Program.
NHTD is designed for eligible adults who require a nursing-home level of care but can live safely in the community with appropriate services and support.
Services may include service coordination, personal care assistance, home and community-based support, assistive technology evaluations and help navigating the NHTD application process.
For some individuals, NHTD can help make it possible to remain at home or transition from an institutional setting back into the community.
TBI Waiver Program
The Traumatic Brain Injury (TBI) Waiver Program supports eligible New Yorkers with traumatic brain injuries or certain related conditions who want to live in the community rather than a nursing facility.
Depending on the approved service plan, TBI Waiver services can include service coordination, independent living skills training, community integration counseling, home and community support services, respite, assistive technology, environmental modifications and other specialized supports.
S&A Unified works with individuals and families navigating these complex care needs and community-based services.
OPWDD and Pediatric Care
S&A Unified provides services for children and adults with qualifying developmental disabilities as well as pediatric clients with complex care needs.
Our OPWDD and pediatric programs are designed around individualized, person-centered support and may involve community habilitation, respite services, family support, care coordination, day services and other supports depending on eligibility and the individual’s approved program.
Our team works with families, care managers and healthcare professionals to help coordinate appropriate support while promoting independence, safety and quality of life.
Is HHA or PCA Care an Alternative to CDPAP?
HHA/PCA and CDPAP can both help eligible people receive assistance at home, but they are not the same program.
CDPAP is consumer-directed. Traditional HHA/PCA care is agency-supported.
For a family that does not specifically need the consumer-directed structure of CDPAP, agency-based home care may be worth exploring. S&A Unified can discuss the home care programs we provide and help determine which next steps may be appropriate based on the individual’s situation.
Eligibility and insurance coverage vary by program. S&A Unified works with many New York Medicaid Managed Long-Term Care plans as well as Medicare and commercial insurance in applicable situations. Coverage should always be confirmed for the specific service and individual plan.
